Diagnostic Delay of Cluster Headache in Korea

Abstract

Background: Cluster headache (CH) is a rare primary headache characterized of extremely painful unilateral pain attacks with cranial autonomic symptoms during cluster bouts. The aim of this study was to investigate diagnostic delay of CH in Korean patients.
Methods: We used a prospective multicenter registry of 15 hospitals in Korea between September 2016 and June 2018. Duration of diagnostic delay was defined as the time interval between the year of the first-onset of CH and the year of CH diagnosis by physician.
Results: Among 191 eligible patients, mean duration of diagnosis delay was 5.5¡¾7.2 years, ranging from 0 to 36 years. Regarding CH subtype, duration of each subtype was significantly different: 0.3¡¾0.8 year for firstonset CH, 7.4¡¾7.8 years for episodic CH, 2.1¡¾2.5 years for chronic CH, and 3.5 years for probable CH (p< 0.001). In terms of the year of onset, diagnostic delay of ¡Ã3 years after their onset was reported in 88% of patients who developed CH between 1979 and 1998, in 77% of patients who developed CH between 1999 and 2010, and in 47% of patients who developed CH between 2011 and 2015.
Conclusion: In Korea, diagnosis of CH was frequently delayed, especially in episodic CH. Therefore, public awareness of CH needs to be increased for reducing its diagnostic delay.
